Students who require a doctor’s note to verify illness are asked to contact their family physician. If you cannot reach your family physician or don’t have one, please contact Health and Counselling Services (HCS).
Missed Coursework and Examinations
Due to medical and other extenuating circumstances, students may occasionally be unable to fulfil the academic requirements of their course(s) in a timely manner. Please review the Academic Consideration Policy to determine if a doctor’s note is required for missed coursework or examinations.
About Doctor’s Notes
Doctor’s Notes are not covered by OHIP or by UHIP, nor are they covered by the CUSA Health & Dental Plan or the GSA’s Health Benefits. Most private insurance providers do not cover this expense. Doctor’s notes are a chargeable service and are $30.
If an illness occurs when the clinic is closed and you get a note from a physician off-campus, ensure it is written on their professional letterhead. Only the attending physician who personally diagnosed the student at the time of their illness can write the note.
You cannot come to Health and Counselling Services and ask for a Doctor’s Note for a past illness. You must see a doctor when you are presently ill for them to make an appropriate diagnosis. It is up to the doctor’s discretion to provide you with a note.
