If you need to cancel and/or reschedule an appointment, we require 24 hours cancellation notice, or a fee will be incurred. Visit the CHR Connect App or through the CHR Connect Website or call 613-520-6674 during business hours.

Fees are based on Ontario Medical Association rates as some services are not covered by OHIP, UHIP and/or private insurance.
